In accordance with the Texas Department of Health immunization schedule, recommendations and adopted by the Texas Board of Health and published in the Texas Register annually, a child or student shall show acceptable evidence of vaccination prior to entry, attendance, or transfer to a child care facility or public or private elementary or secondary school, or institution of higher education. Exclusions from compliance are allowable on an individual basis for medical contraindications, reasons of conscience, including a religious belief, and active duty with the armed forces of the United States. Children and students in these categories must submit evidence for exclusion from compliance as specified in the Health and Safety Code, 161.004(d), Health and Safety Code 161.0041, Education Code, Chapter 38, Education Code, Chapter 51, and the Human Resources Code, Chapter 42.

7th grade Immunization requirements !!
Texas state law requires that all incoming 7th grade students receive two additional immunizations, Tdap and meningitis. These immunizations can be given anytime after the age of 11. Once given, please provide a copy via email or by hand.

Medication Information
All medication must be brought to and picked up at the clinic/front office by a parent or guardian. Students may NOT carry medication to and from school. Any prescription medication brought in must be in the original container with the pharmacy/dosage labels present.
All prescription and non-prescription drugs to be administered or kept at school for longer than 10 days must be accompanied by a written request signed and dated by the prescribing physician and the parent or guardian requesting this service.
Over-the-counter medication may be kept in the clinic for 10 days with a note from the parent, if medication is needed longer a doctor's note will be required.
ASTHMA
Students are not permitted to carry or self-administer their inhaler at school without physician and parental permission.
All inhalers brought to the clinic need to be in their ORIGINAL box with the pharmacy label including the student's name, date of birth, and expiration date.
All students diagnosed with asthma will need to have the following forms completed by their physician and returned to the nurse's clinic each school year.

The Medication Administration Form will also need to be completed by the parent/guardian for EACH inhaler or medication.

ALLERGIES/ANAPHYLAXIS

All students diagnosed with an allergy will need to have the following 2 forms completed by their physician and returned to the nurse's clinic each school year.
The Medication Administration Form will also need to be completed by the parent/guardian for each allergy medication.
SEIZURES
All students diagnosed with a Seizure disorder will need to have the following forms completed by their physician and returned to the nurse's clinic each school year.

Vomiting
Vomiting is not always indicative of a serious illness. Vomiting may occur due to a sensitive gag reflex, overheating, or exercising directly after a meal. Vomiting that is accompanied by other symptoms such as a fever, diarrhea, or upper respiratory symptoms is indicative of an illness. Children who vomit two or more times in a 24-hour period or who present with other symptoms will be sent home. Students should not be sent back to school until they have gone 24 hours without vomiting.
